What to bring to your appointment:

  • Photo ID

  • Your current valid insurance card

  • Referral or Authorization from your PCP, if necessary

  • Your Specialist Co-pay

  • Medical History, including all current medications and dosage information

  • Any previous medical records, x-rays, CT scans, MRI scans or other tests relating to your current medical issue

Print and fill out intake forms prior to your visit!

 

Office Policies :

 

​​​​​Arriving

For your first visit, please arrive at least 15 minutes before your scheduled appointment time in order to complete your registration paperwork.

 

Appointment Changes

If you must reschedule or cancel your orthopaedic appointment, please call us as soon as possible.

 

Prescription Policy

Prescription refills are handled daily from 9:00am to 4:00pm. Please have the name of your medication, dosage and pharmacy number and address available when requesting a refill.  Remember to check your medication requirements prior to the weekend and holidays!

 

Disability and FMLA Paperwork

Please allow at least 48-72 hours for paperwork to be completed.

We participate in a number of health plan networks. Please contact the office directly to discuss insurance benefits and coverage for your specific insurance plan.

If you need a referral, it is your responsibility to obtain a referral from your Primary Care Physician PRIOR to your visit.  Insurance contracts do not allow retroactive referrals. Failure to adhere to the above procedure may deem your appointment your own financial responsibility.
